Building a Healthier Future Through Health Equity and Population Health

At Lakeridge Health, our vision is to create a healthier future together. Achieving that vision means understanding not only the health needs of individual patients, but also the needs of the communities we serve. That's where Health Equity and Population Health play an important role. 

While these terms are often used together, they each bring a unique perspective to improving health outcomes. 

Health equity is about recognizing that not everyone has the same opportunity to achieve their best possible health. Factors such as race, gender, disability, culture, income, where someone lives, and lived experiences can create barriers to care and contribute to differences in health outcomes. Health equity focuses on identifying and addressing those gaps so everyone can access the care and support they need. 

Population health takes a broader view. It looks at the health of entire communities and seeks to understand the factors that influence well-being beyond what happens inside a hospital or clinic. By understanding trends, partnering with communities, and addressing the root causes of poor health outcomes, we can create healthier communities and deliver better care for everyone. 

Together, Health Equity and Population Health help us understand who we serve, where inequities exist, and how we can take meaningful action to improve outcomes across Durham Region. 

At Lakeridge Health, this work begins with listening. Data helps us identify patterns and opportunities, but listening to patients, clients, families, team members, and community partners helps us understand lived experiences and where change is needed most. From there, we work alongside communities to develop solutions, build trust, and take action. 

This commitment is embedded in our strategic priorities and reflected in our Inclusion, Diversity, Equity, Accessibility and Anti-Racism (IDEAA) Multi-Year Action Plan. Since launching the plan in 2023, we have continued to advance initiatives that foster more inclusive, equitable, accessible, and culturally safe environments across our organization. We know that when people feel respected, valued, and experience a sense of belonging, health outcomes improve. 

Key examples include our renewed Accessibility Plan, which helps identify and remove barriers for patients, families, visitors, and team members, and our Indigenous Cultural Practices Policy and Procedures, which support Indigenous patients and families in accessing traditional cultural and spiritual practices as part of their care journey. The policy provides guidance to team members in facilitating requests for practices such as smudging, prayer, drumming, and other ceremonies in a respectful and timely manner. This work is complemented by the creation of culturally responsive spaces across our hospitals, including dedicated Sacred Fire spaces that support healing, connection, and culturally safe care. Together, these initiatives help ensure care is responsive to the diverse needs, identities, and experiences of the communities we serve. 

Our commitment is also demonstrated through targeted initiatives that address specific health inequities. In partnership with the Durham Community Health Centre and the Sickle Cell Awareness Group of Ontario, we are advancing care for people living with sickle cell disease through team member education, community outreach, and screening initiatives. This work helps improve access to care, strengthen awareness, and support more equitable health outcomes for patients and families. 

We are also working alongside communities to improve health and well-being through initiatives such as Black Men's Health Day,the Women's Health Symposium and EmpowerHER. These events provide opportunities for health education, screening, and meaningful dialogue, helping to strengthen trust, increase access to culturally responsive care, and build lasting connections with the communities we serve. 

Health Equity and Population Health are not separate from the work we do every day - they are foundational to delivering exceptional care and achieving our strategic vision of One System.
