Patient Requests
If you would like to request access to your personal health information, (health records) please contact our Release of Information (ROI) Specialists via the contact information below.
For copies of your health records, a fee of $30 applies for up to the first 20 pages. Additional copies will be an additional $0.25 per page.
You can also access your health records through our free, secure, online patient portal, MyChart Central East Ontario. To sign up for MyChart Central East Ontario click here.

Third Party Requests
If you are requesting records for legal or insurance purposes, please mail the signed consent, and a cheque of $33.90 to the mailing address below.
Chart Correction Requests
To submit a request for correction to your personal health information, please contact our Privacy Office at (905) 576-8711, ext. 32757 or ext. 34367.
Form/Letter Completion
If you require assistance with completing forms or obtaining a letter from a physician, please contact your Family Doctor as the first point of support. If this is not successful, you may reach out to the physician’s external community office. Should these options be unsuccessful, please contact the Patient Experience Department for further assistance.
