Need a Family Doctor? Lakeridge Health Is Helping Patients and Community Members Get Started

Still looking for a family doctor? You do not have to keep calling offices yourself. 

Lakeridge Health is helping patients and community members who do not currently have a primary care provider take the first step toward possible connection with a family physician or primary care team in their community. 

Access to a primary care provider, such as a family doctor or nurse practitioner, is an important part of follow-up care, ongoing care and long-term health. A primary care provider can support you with: 

How to Get Started 

If you do not currently have a family doctor in Durham Region, complete one short consent form and then we’ll take it from there.  

This gives Lakeridge Health permission to share your information with participating physicians or primary care teams that may be accepting new patients and explore possible connection opportunities on your behalf. Please note that completing the form does not guarantee immediate connection.  
 
As an alternative, you can also register online for the Health Care Connect list or call 811 to add your name to the provincial list. 

Through this initiative, Lakeridge Health is helping strengthen the connection between hospital and community care so Durham Region residents can access ongoing primary care closer to home.
