Sacred Steps Program Launches to Support Indigenous Foot and Wound Care in Durham Region

A new collaborative initiative is helping Indigenous community members across Durham Region access earlier, more culturally safe care to prevent lower limb complications and improve long-term health outcomes. 

Officially launched in May, Gchi-twaa Dadaakokiid: Sacred Steps is a lower limb preservation program designed to support Indigenous adults who are experiencing, or are at risk for, lower limb wounds and complications related to diabetes, peripheral vascular disease, chronic edema, neuropathy, and other circulatory or mobility concerns. 

The program was developed by the Mississaugas of Scugog Island First Nation and Durham Ontario Family Health Team, in partnership with Durham Community Health Centre, North Durham Family Health Team, Region of Durham Community Paramedics, Lakeridge Health, and Ontario Health atHome. 

Partners came together in response to the significant impact that diabetes, vascular disease and chronic lower limb wounds can have on Indigenous communities, recognizing the need for more accessible care that is rooted in cultural safety and delivered closer to home. 

Sacred Steps focuses on prevention and early intervention through outreach clinics within Indigenous communities, coordinated wound and lower limb assessments, chronic disease management supports, and culturally grounded approaches to care. A dedicated Indigenous Navigator is embedded within the program to help support patients and families, coordinate care, and strengthen cultural safety throughout the care journey. 

The initiative reflects Lakeridge Health’s commitment to advancing health equity through meaningful partnerships and community-based care solutions. By bringing services directly into communities and connecting patients with coordinated supports, Sacred Steps looks to help preserve mobility, wellness, and quality of life. 

Sacred Steps shows the power of collaboration to create innovative, client-centred solutions that respond to the unique needs of communities. Through strong partnerships and a shared commitment to culturally safe care, the program is helping ensure Indigenous patients can access the right care, at the right time, in the right place. 

Who can access the Sacred Steps program? 

The program supports Indigenous adults in Durham Region with lower limb wounds or foot concerns, or who are at risk of complications from chronic disease. Services may include screening, prevention, wound care, education, and coordinated support. 

The program may be appropriate for Indigenous people in Durham Region who: 

  • have diabetes or are at risk of it 

  • are at risk of lower limb or foot complications from chronic disease, diabetes, or vascular disease 

  • have active or recurring lower limb wounds or foot concerns 

  • need wound care, foot care, prevention, or navigation support 

  • face barriers to clinic-based care 

  • would benefit from coordinated, culturally safe, community-based care
