Lakeridge Health 2024/25 Annual Business Plan
Lakeridge Health continues to grow, evolve, and plan for a healthier future for all. Building on a strong foundation, we remain focused on creating a more connected, sustainable, and inclusive health system – one that meets the needs of our communities today while preparing for tomorrow’s challenges and opportunities.
Our 2025/2026 Annual Business Plan sharpens our focus on four key areas: safety, access, people, and financial health. It integrates three important components that guide our work:

Together, these elements align with our current Strategic Plan and lay the groundwork for our next, that will be launched in April 2026. We look forward to continuing this journey with our staff, physicians, volunteers, partners, and community.

2025/2026 Quality Improvement Plan (QIP)
The Quality Improvement Plan (QIP) is an output of the annual business planning process and includes the selection of quality indicators, targets, and change initiatives for improvement for both the acute care and long-term care setting as outlined as a legislated requirement through the Excellent Care for All Act.
The 2025/2026 QIP indicators represent key areas of quality improvement for the organization and are designed to ensure Lakeridge Health is focused on organizational priorities, as well as and planned and/or current initiatives.
The following indicators were approved by the Quality Committee of the Board of Trustees for inclusion in Lakeridge Health’s 2025/2026 QIP:
Hospitals:
- Number of workplace violence incidents resulting in lost time or health care sought.
- Daily average number of patients waiting in the Emergency Department (ED) for an inpatient bed at 8:00 a.m.
- Patient Experience: Did you receive enough information from hospital staff about what to do if you were worried about your condition or treatment after you left the hospital?
- Percentage of front-line staff in ED and surgery who have completed relevant Inclusion, Diversity, Equity, Accessibility, and Anti-Racism education.
Long-Term Care:
- Rate of potentially avoidable ED visits for long-term care residents.
- Percentage of long-term care residents who fell in the last 30 days.
- Percentage of residents with a worsened stage 2-4 pressure ulcer.
- Percentage of residents who responded positively to having a voice in participating in care planning decisions and services received.
Provincial Wait Times (Surgery, Diagnostic Imaging & Emergency Department)
Lakeridge Health reports performance against a number of wait time measures to Ontario Health via Health Quality Ontario (HQO) & Access to Care (ATC). These measures demonstrate how Lakeridge is performing with respect to wait times for surgeries and procedures, diagnostic imaging tests (MRI & CT) and the time spent in emergency departments.

National Health System Performance
The Canadian Institute for Health Information (CIHI) produces a national tool for reporting on the performance of the Canadian health system & the health of Canadians. The health system is measured across five themes: Access, Quality of Care, Spending, Health Promotion & Disease Prevention, Health Outcomes.
Information is available for Lakeridge Health, as well as other hospitals, regional health areas, provinces and at a national level.
