Diabetes Care

Diabetes is a condition in which the body either cannot produce insulin or cannot properly use the insulin it produces. In 2024, Diabetes Canada reported that 31% of residents in Ontario live with diabetes or prediabetes.   

 There are three major types of diabetes: 

  •  Type 1 diabetes  
  •  Type 2 diabetes 

  •  Gestational diabetes (occurs during pregnancy and is usually temporary) 

Prediabetes is another important diagnosis that indicates an elevated risk of developing diabetes.  

Diabetes-related complications can be very serious and even life-threatening. Properly managing blood sugar levels reduces the risk of developing these complications.

We work closely with adults, giving them the information and resources they need to manage their health and live well with diabetes. In addition to providing diabetes education, we are also the site of one of three Centres for Complex Diabetes Care in the Central East Local Health Integration Network (LHIN).  

Diabetes Education Program

We provide individual assessment and nutrition counselling, insulin and blood glucose meter instruction, and other group education sessions related to talking about diabetes and making lifestyle changes. 

Our locations: 

Centre for Complex Diabetes Care (CCDC)

Approximately 15% of all diabetes patients have complex health issues and require intensive services for a short period to stabilize their health.

The CCDC was established to assess and support clients with diabetes who have complex needs, such as co-morbid health conditions that require intensive case management. The CCDC offers a single point of access to a specialized inter-professional team. It is a care delivery model for people with diabetes who need more contact, more resources and more follow-up across health care and social services systems. Care is provided over a shorter term, patient-centred, goal specific and allows for frequent follow-up.

The team consists of the following experienced registered professionals:  

  • Social Worker

  • Registered Nurse  

  • Registered Dietitian

What to bring to your appointments 

Be sure to bring the following items to each appointment: 

  • Your health card 

  • Your blood glucose meter and log book 

  • A list of all your medications and supplements
