Durham Mental Health Services - Release From Custody Program

The Release From Custody Program has the following goals:

• To partner with mental health court workers and local correctional facilities for appropriate discharge planning
• To provide a central point of contact for individuals to access/be referred to mental health services and other required supports
• To coordinate a continuum of services based and client choice and need
• Comprehensive individualized assessment and planning
• Systemic advocacy and coordination
