Lakeridge Health at Home

Lakeridge Health has partnered with Bayshore HealthCare to expand healthcare services beyond the hospital.  The Lakeridge Health at Home (LH@Home) program delivers time-limited restorative support to patients when discharged from hospital.  The enables patients to continue their recovery journey and access rehabilitative care in their own home.
