Reactivation Program

Lakeridge Health has partnered with Bayshore HealthCare to offer the Reactivation Program at the Transitional Care Unit located within Traditions of Durham.  This time-limited program supports patients, who no longer need to be in hospital, access to rehabiliative care.  The Reactivation Program enables patients to continue their recovery journey and access rehabilitative care in preparation for the transition home.     

The interprofessional team at the Reactivation Program includes nurses, personal support workers, physiotherapists, occupational therapists, rehabiliative assistances, dietitians, and recreation therapists. Together with an interprofessional care team, patients, families and/or care partners develop and implement a personalized care plan to achieve a seamless transition home.
