Geriatric Clinic

The Lakeridge Health Oshawa Geriatric Assessment and Intervention Network (GAIN) Clinic is one of twelve hospital and community-based clinics that provide interprofessional, comprehensive geriatric assessments (CGA) and interventions for older adults, aged 65 or older, with complex health concerns living in the community. The team works collaboratively with patients, family, care partners, and their primary care providers to strengthen supports and optimize the function of community-dwelling older adults.

The CGA is supported by interprofessional care teams that may include nurse practitioners, geriatric specialist physicians (i.e. Geriatric medicine and Care of the Elderly), nurses, dedicated Behavioural Supports Ontario (BSO) nurses, physiotherapists, occupational therapists, and pharmacists. This interprofessional team is skilled in goal-based care planning and interventions for older adults living with frailty and cognitive impairment.

The CGA is intended to address challenges associated with aging that negatively impact function and independence, such as:

• Burden of care;
• Cognition;
• Continence;
• Medications;
• Mobility;
• Nutrition;
• Pain;
• Sleep; and
• Social History.

How to Refer 

Referrals are accepted from primary care providers, hospitals, community organizations, individuals and care partners via Ocean eReferral Network.  The Ocean eReferral Network is a Ministry of Health funded initiative to simplify and streamline the referral management process.

Being Dropped Off by Car

Use the Dialysis Drop-Off area on Parkwood Court. Enter through the Dialysis Drop-Off doors, and you will be in the GAIN clinic.

If You are Driving

The closest parking garage is the South Garage on Parkwood Court. From the parking garage, walk East up Parkwood Court until you see the Dialysis Drop-Off entrance on your left. Enter through the doors and you will be in the GAIN Clinic.
