Seniors and Complex Continuing Care


Complex Continuing Care (CCC) is a temporary care setting to support patients with complex and non-urgent medical care needs transition to their next stage of care.  This step of the patient’s recovery journey is to optimize their quality of life and learn how to manage their condition before moving to their next care setting home  

The interprofessional care team includes nurses, personal support workers, physiotherapists, occupational therapists, social workers, dietitians, physician, pharmacists, recreation therapists, and spiritual health practitioners.  Together with patients, families, and care partners, the interprofessional care team develops and implements an individualized care plan to prepare for transition to the next stage of care. 

Co-payment will apply to all patients as per the Ministry of Health.
