About the Department of Family and Community Medicine

The Department of Family and Community Medicine at Lakeridge Health connects Primary Care and Hospital Care across Durham Region. We work alongside family physicians, nurse practitioners, specialists, hospital teams, and community partners to strengthen communication, improve care coordination, and support better outcomes for patients.

Our focus is simple: making it easier for Primary Care Providers (PCPs) to navigate the healthcare system, access hospital resources, and deliver connected care for their patients.

As healthcare becomes increasingly complex, no provider should feel they are working in isolation. The DFCM helps bridge gaps between sectors so that patients experience a more seamless healthcare journey, whether they are receiving care in the community, in hospital, or transitioning between the two.
