Our Partners

The Department of Family and Community Medicine (DFCM) operates within Lakeridge Health and is embedded within the Primary Care Network of the Durham Ontario Health Team (OHT).

The Durham OHT brings together healthcare organizations, providers, patients, and community partners to improve access to coordinated care across the healthcare system. This includes Primary Care, hospitals, mental health and addiction services, home and community care, long-term care, and social support services.

Within this network, the DFCM serves as a connector between hospital-based care and community-based care, helping providers work more effectively together in support of shared patients.

We are also proud partners of the Queen’s Family Medicine Program at Lakeridge Health. This growing program supports the training of future family physicians within Durham Region and helps strengthen the long-term sustainability of Primary Care in our communities.
