Our Story
The Department of Family and Community Medicine (DFCM) was created in response to one of the most challenging periods in modern healthcare: the COVID-19 pandemic.
During the pandemic, hospitals and Primary Care Providers across Durham Region worked together in new and unprecedented ways. Information, resources, expertise, and clinical support were shared rapidly to meet the needs of patients and communities. That experience demonstrated what is possible when healthcare providers work as one connected system.
The DFCM was established to build on that momentum and create a stronger, more coordinated relationship between Primary Care and Acute Care for the future.
Today, that same spirit of collaboration continues to guide our work.
