Our Vision for the Future

We envision a healthcare system where Primary Care Providers are fully connected to the broader health system and supported by integrated, team-based care.

Our long-term goal is to help build a “medical neighbourhood” model across Durham Region, where patients and their Primary Care Providers are at the center of care and surrounded by coordinated teams of healthcare professionals, services, and supports.

Today, many Primary Care Providers continue to work independently or in small practices with limited access to interdisciplinary resources. At the same time, patients often face challenges navigating a fragmented healthcare system.

The DFCM is working to help change that.

By strengthening relationships between Primary Care, hospitals, and community services, we aim to improve access, reduce barriers, and create a more connected healthcare experience for both providers and patients.

Our vision is a future where every resident of Durham Region has equitable access to high-quality, team-based care close to home.
